This privacy policy has been compiled to better serve those who are concerned with how their 'Personally identifiable information' (PII) is being used online. PII, as used in US privacy law and information security, is information that can be used on its own or with other information to identify, contact, or locate a single person, or to identify an individual in context. Please read our privacy policy carefully to get a clear understanding of how we collect, use, protect or otherwise handle your Personally Identifiable Information in accordance with our webapp.
This app was created by and is the property of RemedyEHR LLC. for the implicit use of Transformations Comprehensive Weight Loss & Management at River Oaks Hospital and its patients or potential patients. We reserve the right to revoke or restrict the use of this app from any of the above or other parties for any reason deemed necessary such as in the case of property or software abuse or nonadherence with our TOS/Licensing agreement.
The sections below explain how RemedyEHR in conjunction with Transformations Weight Loss collects, uses and protects patients and or potential patients Personally Identifiable Information.
What personal information do we collect from the people that visit our site, webapp or app?
When ordering or registering on our app, as appropriate, you may be asked to enter your name, email address, phone number or other details to help you with your experience.

We collect information from you when you fill out a form or enter information on our app.

We do include links to Transformations at River Oaks website and the information provided there.
We also include links to RemedyEHR.com website and the MyHealthManager webapp created by RemedyEHR for patients at Transformations.
We have not included links to any other third party application or site and will not without the consent of Transformations.

We have not enabled Google AdSense on our app but we may do so in the future.
California Online Privacy Protection Act
CalOPPA is the first state law in the nation to require commercial webapps and online services to post a privacy policy. Once this privacy policy is created, we will add a link to it on our home page, or as a minimum on the first significant page after entering our webapp.

How does our app handle do not track signals?
